Purpose: To provide Couriers an opportunity to opt out of mandatory arbitration provisions under the governing agreements, in compliance with applicable law.
Courier may be required under the Courier Independent Contractor Agreement and other Platform agreements to resolve disputes through arbitration rather than litigation. Arbitration is binding and may limit certain rights.
Courier may opt out of this provision by completing and submitting this form within the time frame specified by the Platform.
Courier acknowledges that arbitration:
Replaces jury trials and court proceedings for most disputes
Is conducted under rules established by the governing arbitration body
May limit the ability to join class actions or collective claims
Is final and binding except in limited circumstances
To opt out:
Complete all fields on this form accurately
Submit via the designated Platform submission method (email, portal, or certified mail)
Submission must occur within 30 days of account creation or acceptance of the governing agreements
Failure to timely submit the opt-out form will result in the mandatory arbitration provision remaining in effect.
Courier certifies that:
They have read and understood the arbitration provisions
They voluntarily choose to opt out
They understand that opting out does not affect any other contractual obligations
Courier signature (electronic or handwritten) confirms acknowledgment and election.
